Public Burden Statement: A federal agency may not conduct or sponsor, and a person is not required to
respond to, nor shall a person be subject to a penalty for failure to comply with a collection of information subject
to the requirements of the Paperwork Reduction Act unless that collection of information displays a currently
valid OMB Control Number. The OMB Control Number for this information collection is 21XX-XXXX. Public

the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed,

Instructions:
The purpose of this form is to provide an optional set of basic community engagement survey questions for
BEYOND and Partnership for Safety Plan (PSP) participants. These questions exemplify the baseline
information that the Federal Aviation Administration (FAA) would like to receive regarding community sentiment
about unmanned aircraft system (UAS) operations. Participants may use all, some, or none of these questions
when designing their surveys. Each participant should build their own surveys tailored to their individual concept
of operations. A participant with multiple UAS operations (package deliveries, infrastructure inspections, etc.)
should have multiple surveys, one tailored to each operation. The objective of the surveys is to identify best
practices and lessons learned to inform ongoing and future stakeholder engagement activities. The surveys are
not meant to be statistical in nature, nor to provide statistically significant information for policy or decisionmaking.
In alignment with the BEYOND Memoranda of Agreement (MOA) and the PSP Memoranda of Understanding
(MOU), BEYOND and PSP participants should provide to the FAA the raw survey results, their analysis of the
survey results, and any adjusting actions taken based on the analysis.
