Dear ART Program Staff:

This letter contains important information regarding 20xx Assisted Reproductive Technology (ART) data reporting to the
Centers for Disease Control and Prevention (CDC) as required by with the Fertility Clinic Success Rates and Certification
Act of 1992 (42 U.S.C. 263a-1). Please share this with everyone in your program who is involved in the process. The
deadline for reporting data to the CDC on ART cycles started in 20xx is December 15, 20xx.
The above deadline is for online submission of 20xx ART data through the National ART Surveillance System (NASS
2.0) at http://artreporting.cdc.gov and includes reporting of all ART cycles that did not proceed to oocyte retrieval or
transfer for any reason, as well as all ART cycles proceeding to retrieval and transfer. All Society for Assisted
Reproductive Technology (SART)-member clinics must complete CDC submission requirements through NASS 2.0 in
addition to any SART reporting requirements.
ART clinics in operation at any time during 20xx that fail to submit required 20xx data to CDC through NASS 2.0 by
December 15, 20xx, will be listed as nonreporters in CDC’s 20xx ART Fertility Clinic Success Rates Report. In addition,
each nonreporter will have an individual page with the clinic’s name but no ART data included as part of the fertility
clinic table section of the report.
Your ART clinic needs an account to access the NASS website and submit data to CDC. Medical Directors will verify
clinic table data via electronic signature when completing their NASS online submission. Existing NASS 2.0 accounts can
be used to report 20xx data for the same clinic. Please contact the NASS Help Desk at 1-888-650-0822 or by e-mail at
NASS@Westat.com if your clinic has not been initialized in NASS 2.0, or if you need additional assistance. If there have
been any changes in your clinic’s operation, location, or key staff, please notify the NASS Help Desk. As a reminder,
clinics should not mail in signed hardcopies of the clinic tables or edit reports. For more information about the NASS 2.0
submission process for 20xx data, refer to the submission guide posted in the Definitions & Manuals menu item on the
NASS 2.0 website (http://artreporting.cdc.gov). To access the submission page, log into NASS 2.0 and select the 20xx
reporting year in the Submit or View Annual Data section on the Main Page.
Advance Notice about Reporting Year 20xx Laboratory Accreditation
In consultation with the American Society for Reproductive Medicine and SART, CDC reports the accreditation status of
all ART programs as a public service and publishes this information in the CDC’s annual report. CDC does not endorse
accreditation agencies. Based on earlier recommendations by ASRM and SART, CDC previously accepted laboratory
accreditation by at least one of three agencies: the College of American Pathologists (CAP), the Joint Commission (TJC),
or the New York State Tissue Bank Program (NYSTB). Based on new recommendations by ASRM and SART, laboratory
accreditation will only be accepted from CAP and TJC effective for the 20xx data submission (deadline for reporting by
December 15, 20xx). If your embryo laboratory is currently accredited by NYSTB only, you will need to apply for
laboratory accreditation by either CAP or TJC. Without proof of accreditation from CAP or TJC in 20xx, your clinic will
be listed as having no laboratory accreditation in the CDC’s 20xx ART Fertility Clinic Success Rates Report.
