Recently, you were listed as a supervisor of Shawn Wheeler who completed the National Fire Academy
(NFA) course: F0457 — Decision-Making for Initial Company Operations 10/10/2020 — 10/11/2020 .
To help guide the NFA in the continuous improvement of its training programs, we are asking you to
please complete a Long Term Evaluation (LTE) of this course. Your feedback is critical for the Academy. This
will assist us in determining what training elements have been transferred to the job and, ultimately, are
making a difference in helping to reduce the loss of life from fire-related incidents in local communities.
