Applicants answering “Yes” to the question “Do you intent to file for SSI” when applying for Disability benefits, will
receive this page. This page provides SSI Closeout language and additional information to the applicant regarding the
application. We updated the language to indicate that a SSA technician will contact the applicant to obtain the SSI
application. This screen is provided to 1st and 3rd party applicants, both versions provide the same information but uses
different pronounces e.g., “You told us that “John Public” wanted …”
