The purpose of this survey is to provide the Administration for Native Americans with information on your experiences with combating COVID-19 and adapting. Your participation is voluntary, and the information provided will be kept private.

Public reporting burden for this collection of information is estimated to average 2 minutes per response, including the time for reviewing instructions, gathering and maintaining the data needed, and reviewing the collection of information. This is a voluntary collection of information.
