Minor Revisions to the SSA-1383
SSA is making the following revisions:
Change #1: In question #1, we are revising the instructions to advise the person completing the form to print new address, if any, on page 2 of the form.
Justification #1: Currently, the instructions are to print new address at bottom of form, but there is no place at the bottom of the form to do so. Since the address portion of the form is on page 2, we are making this change for clarity reasons.
Change #2: In question #2b, we removing the language “FILL IN BOTH BOXES” and unbolding the word “AMOUNT.”
Justification #2: “FILL IN BOTH BOXES” is a carryover from older versions of this form, and it no longer applies. We are unbolding “AMOUNT” to be consistent with the look of the response for #2a.
Change #3: In question #6d, we are bolding “STUDENT’S SOCIAL SECURITY NUMBER.”
Justification #3: We are bolding this label to be consistent with the other labels in #6a, b, c, d, and e.
Change #4: In question #8, we are removing the word “a.”
Justification #4: We are removing the word “a” for grammatical reasons.
Change #5: On page 2, under HOW TO REPORT, in the NOTE, we are replacing the period with a comma.
Justification #5: We are correcting for grammatical reasons.
Change #6: On page 2, under “WHAT TO REPORT,” we are replacing “the reverse side” with “page 1.”
Justification #6:
We are correcting to use the page number in case the form is not
printed double‑sided.
Change #7: We are revising the PRA statement on this form.
Justification #7: We are revising the PRA statement to reflect our current boilerplate language. The current language, which dates back to the last reprint of the form, is now outdated.
Change #8: We are revising the Privacy Act Statement on this form.
Justification #8: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the form.
Minor Revisions to the SSA-1383-FC
SSA is making the following revisions:
Change #1: In the title, we are revising the language in the instructions to eliminate passive voice.
Justification #1: We changed the language to comply with the agency guidelines to use active voice.
Change #2: In the second block on the form, we are revising the language in the second sentence of the instructions to eliminate passive voice.
Justification #2: We changed the language to comply with the agency guidelines to use active voice.
Change #3: In question #1, we are revising the instructions to advise the person completing the form to print new address, if any, on page 2 of the form.
Justification #3: We made this change for consistency purposes.
Change #4: In question #8, we changed the language as recommended by the prisoner/fugitive felony policy requirements.
Justification #4:
The Social Security Act was amended in 04/2000 to change the
requirements for prisoner suspension.
Change #5: We are revising the PRA statement on this form.
Justification #5: We are revising the PRA statement to reflect our current boilerplate language. The current language, which dates back to the last reprint of the form, is now outdated.
