The goal of this information
collection is to facilitate a CDC public health mission as provided
under the PHSA and Code of Federal Regulations. Collected
information is intended to prevent the spread of communicable
disease into and within the United States. The change request
submitted in April 2020 adds a new cruise ship reporting form that
will solicit data concerning COVID-19 cases or potential COVID-19
cases.
