We recently sent you an email asking you to participate in a survey (the Introductory Session Landscape Assessment) about your CDC-recognized National Diabetes Prevention Program (DPP) lifestyle change program (e.g., introductory session characteristics, recruitment strategies) and characteristics of your community.

The survey will take only a few minutes of your time and will help provide the CDC with a better understanding of the types of introductory sessions (also known as “session zeros”) being offered as a recruitment strategy to increase enrollment in the National DPP lifestyle change program.
