Attachment E – Tablet Screening Video Scripts

NSDUH INTRODUCTION VIDEO: VOICEOVER AUDIO - ENGLISH
To serve people across the United States, the National Survey on Drug Use and Health collects
data on substance use, mental health and other health-related issues that are used by local,
state, and national agencies to support education, treatment and prevention programs.
For this survey, households nationwide just like yours are scientifically selected at random and
then visited by a professional interviewer.
During this visit, the interviewer asks a few general questions to determine if anyone in your
household will complete an interview. Those completing the full interview will receive $30 cash.
To protect your privacy, you record most of your answers into a computer. The interviewer
never sees or hears them. Your answers are combined with those from thousands of others,
and are reported only as overall statistics.
You cannot be replaced so your participation is very important.

Para servir a las personas en los Estados Unidos, la Encuesta Nacional sobre la Salud y el
Consumo de Drogas recopila datos sobre el uso de sustancias, la salud mental y otros temas
relacionados con la salud que las agencias locales, estatales y federales utilizan para apoyar
los programas educativos, de tratamiento y de prevención.
Para esta encuesta, hogares exactamente como el suyo se seleccionan al azar en todo el país
mediante el uso de métodos científicos y luego un entrevistador profesional visita estos
hogares.
Durante la visita, el entrevistador hace algunas preguntas generales para determinar si alguien
en su hogar completará una entrevista. Las personas que completen la entrevista en su
totalidad recibirán $30 dólares en efectivo.

Hello, I’m ________with RTI International. We are conducting a
nationwide study sponsored by the U.S. Department of Health and
Human Services. You should have received a letter about this
study. (SHOW LEAD LETTER, IF NECESSARY.)

ONE YOUTH IS SELECTED:
Your (AGE) year-old child has been selected for an interview. I
would like to talk with him/her to see if he/she is interested in
participating in this study which asks about tobacco, alcohol,
and drug use or non-use and other health related issues.
If your child is interested, I will give both of you more information
and ask for permission to complete the interview. (Is he/she
available?)

TWO YOUTHS ARE SELECTED:
Your (AGE) year-old and (AGE) year-old children have been
selected for an interview. I would like to talk with them to see if
they are interested in participating in this study which asks
