FAADroneZone / Registration
The authority for collecting personally identifiable information (PII) through the FAADroneZone website for purposes of registering a small (s)UAS is contained in 49 U.S.C.
44101–44106 and 44110–44113 which require aircraft to be registered as a condition of operation and establish the requirements for registration and registration processes.
Pursuant to 49 U.S.C. 44807 and 14 C.F.R. § 107.13, persons operating small unmanned aircraft under 14 C.F.R. part 107 must comply with these registration requirements.
Additionally, persons operating in accordance with the statutory exception for limited recreational operations of unmanned aircraft under 49 U.S.C. 44809 must comply with
these registration requirements pursuant to subsection (a)(8) of that section. The principal purpose for which information collected is intended to be used is to complete the
aircraft registration process and identify the aircraft to its owner. Disclosure of this information is mandatory, and operation of a sUAS in violation of the applicable registration
rules may result in enforcement action and/or civil penalties pursuant to 14 C.F.R. Part 13. Additionally, the failure to provide the required information will prevent the FAA from
registering your aircraft which is required under 49 U.S.C. 44101 to be completed prior to operation of the aircraft. The information collected to complete the aircraft
registration process and issue the unique identifier is included in a Privacy Act System of Records known as Department of Transportation (DOT)/FAA 801, titled “Aviation
Registration Records.” Records from this system of records may be disclosed in accordance with the routine uses that appear in /DOT/FAA 801, as published in the Federal
Register at 81 Federal Register 54187 (August 15 2016), available at https://www.transportation.gov/individuals/privacy/privacy-act-system-records-notices.
Part 107 Waiver / Airspace Authorization Request Form
The authority for collecting the information you provide on this form is codified at 49 U.S.C. § 44701; see also FAA Modernization and Reform Act of 2012, Pub. L. 112-95 § 333,
Special Rules for Certain Unmanned Aircraft Systems and 14 C.F.R. Part 107. The principal purpose for which the FAA intends to use the information is to receive, evaluate, and
respond to requests for authorization to operate a sUAS pursuant to 14 C.F.R. Part 107, in Class B, C, or D airspace or within the lateral boundaries of the surface area of Class E
airspace designated for an airport, and evaluate requests for certificate of waiver to deviate safely from one or more sUAS operational requirements specified in Part 107. The
information may include, but is not limited to, name, address, phone number, aircraft registration number, and a description of planned operation. Disclosure of this
information is voluntary, but operation of a sUAS outside of the regulations at 14 C.F.R. Part 107 without a waiver or authorization in violation of that regulation may result in an
enforcement action and/or civil penalties under 14 C.F.R. Part 13. Additionally, failure to provide all required information will result in the FAA being unable to issue a waiver or
authorization for your operation under 14 C.F.R. Part 107, subpart D. The information collected on this website will be included in a Privacy Act System of Records known as
DOT/FAA 854, titled “requests for Waivers and Authorizations under 14 C.F.R. Part 107.” Records from this system of records may be disclosed in accordance with the routine

Part 107 Accident Reporting Form
The authority for collecting the information provided on this form is codified at 49 U.S.C. § 44701; see also FAA Modernization and Reform Act of 2012, Pub. L. 112-95 § 333,
Special Rules for Certain Unmanned Aircraft Systems, and 14 C.F.R. § 107.9. The principal purpose for which the FAA intends to use the information is to receive and evaluate
accident reports submitted to the FAA involving sUASs. The Federal Aviation Administration will use the information provided in connection to accident reports to regulate and
promote safety in air commerce, in accordance with its statutory authority at 49 U.S.C § 44701. Disclosure of this information is mandatory for remote pilots in command
operating under 14 C.F.R. Part 107, and a certified remote pilot in command’s failure to report an accident in accordance with the regulations may result in enforcement action
and/or civil penalties pursuant 14 C.F.R. Part 13. Additionally, failure to provide all the information requested will delay the processing of the report. The information collected
