Hello NCCCP Grantee or Partner,
You have been
identified as a NCCCP DP15-1501 grantee or partner working on the
Cancer Survivorship program in your state. We are requesting that
you follow the link in this email and complete a survey about your
efforts implementing cancer survivorship interventions and
strategies. The purpose of this survey is to assess the
implementation and short-term outcomes of the Cancer Survivorship
program across all DP15-1501 grantees.
Completion of
this survey is strongly encouraged. It will help CDC’s Division
of Cancer Prevention and Control learn what is working well and what
is challenging; this information can help inform the roll-out of the
Cancer Survivorship program to all NCCCP grantees. Your responses
will be maintained in a secure manner by our contractor, RTI
International. Individually identifiable information will not
be provided to DCPC staff, and survey responses will be reported in
aggregate.
