terminated in 2015, federal agencies continued to use the form and
did not report burden hours. Therefore, Grants.gov did not have a
baseline of previous burden hours to use in the calculations for
this ICR. Grants.gov used the calculation of 3,467 based on the
number of submissions of the form for FY2017.

Form associated with this ICR automatically after 5 calendar days
from the date the agency's RCF including the common form was
received in OIRA.
This form is used by multiple Federal agencies
in grant applications. Burden hours shall be reported only for HHS.

hours.
