Non-Substantive Change

VA requests approval of a non-substantive revision to OMB control number 2900-0099 VA Form 22-5495 [Dependents Request for Change of Program or Place of Training].
To implement section 308 of Public Law 115-48, a check box is required to allow impacted beneficiaries the ability to elect not to provide educational institutions information regarding their amount of educational assistance entitlement.
Beginning August 1, 2018 VA will be providing educational institutions with information concerning the amount of educational assistance veterans and other eligible individuals are receiving from VA. This will be provided to the institution where the individual is enrolled and pursing his or her education. VA is required to provide this information to the educational institutions, unless an individual elects to not provide information to the educational institution. As such, this revision request for (VA Form 22-5495) is being submitted to include a check box for individuals to elect not to have their information shared with educational institutions.
In order to include the check box, an additional field will be inserted at the top of the application before PART I- APPLICATION INFROMATION. The field will read as follows:
Request to Opt-Out of Information Sharing With Educational Institutions
By checking the box, I CERTIFY THAT THE DEPARTMENT OF VETERANS AFFAIRS (VA) does not have my permission to share information about my veterans’ education benefits with any educational institution. I understand that sharing my information with my school is intended to support the certification process and that “opting-out” may delay that process. See information and Instructions on Page 4 for more information.
We are also adding a revision to Request to Opt-Out of Information Sharing With Education Institutions of the “instructions page” by adding the following paragraph after “withhold this information instead.”:
By checking this box, you CERTIFY THAT VA does not have your permission, to share your personally identifiable information or information about your veterans' education benefits with any educational institution. You are asserting your rights under 38 U.S.C. 3699A(b) to opt-out of any sharing of this information by the Department of Veterans Affair
