Title of Information Collection
Purpose
The purpose of the survey is to gather customer feedback regarding their experience in order to improve the overall experience for
shoppers.
In lieu of the skip logic text entry question asking "Why don't you shop at the commissary more frequently" respondents will be asked
to "Please select up to three of the most significant barriers that prevent or discourage you from shopping at the commissary more
frequently." Three questions were added to provide additional context and insights around the data being collected and are focused
on perceived savings, travel distance and sponsor's grade (skip logic). Responses to questions addressing private label, commissary
spend, item availability and employee interaction were refined to clarify response options and ensure language used is in terms the
respondent more easily understands. Additionally, questions addressing non-commissary grocery purchases and children's ages
were changed to skip logic to reduce survey length and ensure only relevant questions are being asked of respondents.
Respondents will now be asked "Would you like to be contacted for a response to your comment regarding your shopping
experience?" when providing a text entry response to the question "What else would you like to share with us to help improve your
commissary shopping experience?". Contact information will not be retained within the ForeSee survey platform. An email will
automatically be generated to the DeCA Customer Comment forum (customerservice@deca.mil) to include the customer's survey
responses. The customer will receive a response as requested via DeCA's Customer Relationship Management (CRM) forum.

Instructions for completing Request for Approval under the
“E-Government Website Customer Satisfaction Surveys”
Title of Information Collection: Provide the name of the collection that is the subject of the request. (e.g., Comment card for soliciting
feedback on xxxx)
Purpose: Provide a brief description and how you will use this information collection. If this is part of a larger study or effort, please
include this in your explanation.
Description of Respondents: Provide a brief description of the targeted group or groups for this collection of information. These
groups must have experience with the program.
Type of Collection: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form
for each instrument.
Certification: Please read the certification carefully. If you incorrectly certify, OMB will return the collection as improperly submitted or
