Agency Form Number affected by Change Worksheet: Foodborne Illness Outbreak Investigation Survey

Summary of Changes:
FSIS is requesting a non-substantive change to the recently approved
Foodborne Illness Outbreak Investigation Survey. There is no new burden associated with this change. Three minor edits to the survey have been suggested in order to enter the survey into the survey software.

Specifically, in question 3, FSIS would like to add “In addition to interactions you already have with FSIS” to the question, change “education” to “information” under response option “f,” and add a response option “g” for “none.”

Below is a summary of the changes to survey question 3:

In addition to interactions you already have with FSIS, in which of the following ways can FSIS assist your state in responding to future foodborne illness outbreaks that may involve an FSIS-regulated product? (select all that apply)

(Responses A-F will be presented in random order)

    1. Provide information on FSIS isolates (e.g. FSIS-regulated establishment information for an isolate that matches an illness cluster by PFGE)

    2. Assistance with traceback

    3. Assistance with product sampling

    4. Assistance with public information releases or media requests

    5. Assistance with recall verification activities

    6. Informationabout FSIS outbreak investigation procedures

    7. None

    8. Other (open text field)

Additionally, FSIS would like to add an open text box below the response options to question 2 so respondents can add additional comments.
