Introduction
In support of the E‐Enterprise for the Environment, Quality Assurance Project Plan (QAPP) Initiative,  please complete this short 
customer survey. This initiative follows up on actions decided upon at a Lean Event intended to improve timeliness, consistency, 
and transparency. Your feedback will assist us in better understanding our state and tribal partners’ training and resource needs 
for QAPP development and review, contribute to developing QAPP training content, and aid in our improvement of QAPP 
processes.

Instructions for using this Workbook
1. This Workbook consists of two Worksheets: Instructions and Survey Questions
2. Please select the "Survey Questions" Worksheet: The purpose of this Worksheet is to collect information with regards to QA training. This 
Worksheet consists of eight questions. All of the response cells have a "drop down" or "picklist" and the column labeled, "Comments" allows 
for you to include additional information. For the picklist questions:  Click in the cell and a small gray downward pointing arrow will appear.
