SUBJECT: CDC-Supported Survey for Colorectal Cancer Screening Program
Dear [Clinic Liaison Name],
Your clinic is working closely with the state or tribal health department on a special program focused on practice improvements to increase colorectal cancer (CRC) screening rates for your patients age 50-75. This program is called the Colorectal Cancer Control Program (CRCCP) and is supported by the Centers for Disease Control (CDC).
CDC, in collaboration with the University of Washington (UW), is conducting an assessment to learn more about how you have implemented CRC screening practice improvements (also called evidence-based interventions, or EBIs) in your clinic to increase CRC screening.
The CDC Colorectal Cancer Control Program Clinic Survey
You are invited to respond to this survey on behalf of your clinic, as your state or tribal health department has identified you as their clinic collaborator, and the person most knowledgeable about your clinic’s efforts to increase CRC screening at your clinic. In two days, you will receive a link to access and complete the survey, which will come from the University of Washington.
Here are some important details about the survey you need to know:
CDC has been working with UW to develop this survey, and the UW will field this survey on CDC’s behalf.
The person at each clinic who is most familiar with CRC screening practice improvements should complete this survey, and it is okay to consult with others on your team to complete the survey.
The survey should take about 20 minutes to complete; responses will be kept secure.
After completion, you will be given the option to download your responses in pdf form.
When the link arrives, please respond to this survey by [date TBD pending OMB approval].
Why your response is important
CDC will use these data to better describe implementation sites and understand EBI implementation, including the factors that contribute to or hinder EBI implementation. Information collection will inform the development of CDC guidance and resource tools, as well as tailored technical assistance activities, to support future CRCCP implementation with the goal of increasing CRC screening rates over time.
