SUBJECT : Changes to Collection Forms for the Survey of
Occupational Injuries and Illnesses

In December 2017, the Bureau of Labor Statistics (BLS) seeks approval to begin using revised collection forms for the Survey of Occupational Injuries and Illnesses (SOII). Minor corrections and updates made to collection forms to clarify instructions for reporting include:

• Modified wording on prenotification forms clarify that race and ethnicity are optional fields.

  • Wording of the second sentence of the second bullet on the second page is clarified:

    • Old language: In addition, please keep records on the race and/or ethnicity of your injured or ill workers. This information will be requested in January 2018.

    • New language: In addition, this survey will ask for optional race and/or ethnicity information that is not included on the OSHA forms.
