Dear Resident:
On behalf of the U.S. Census Bureau, thank you for your participation in the Survey of Income and
Program Participation (SIPP). We realize that your household has given a great deal of time toward the
collection of the survey data, and we sincerely appreciate your help.
We are using the information from SIPP to better understand and evaluate the economic status of people
living in the United States — especially how things change over time. Policymakers are using the statistics
you helped create to make better economic decisions and enhance the programs that serve our Nation.
The accuracy of the data and success of the survey are only possible through the dedicated participation
of you and others like you.
As a token of our appreciation, we are sending your household a $40 debit card, which you should
receive in a few days. The card will be active upon receipt. We recommend using the card at an ATM
within 30 days of receipt and withdrawing the entire amount in one transaction. The debit card will arrive
with detailed procedures for accessing funds from an ATM. Please read all of the instructions
completely before using the card.
Please note: The PIN for the debit card is the last 4 digits of the 16-digit card number.

For security reasons, our Field Representatives do not carry incentive debit cards on their person. Please
do not contact your Field Representative for assistance, as he or she does not have access to information
about the card. If you do not receive the debit card within 7 business days or have trouble redeeming it,
please call 1–888–369–3608 and we will be happy to assist you.
