This data collection provides
instructions and forms necessary for States to report the number of
children receiving early intervention services under Part C of
Individuals with Disabilities Education Act (IDEA), the settings in
which these children are provided services, and the reasons by
which these children exit Part C of IDEA. The form satisfies
reporting requirements and is used by OSEP to monitor State
agencies and for Congressional reporting.
There is an adjustment decrease
of -1386 annual burden hours; the reported burden has decreased due
to a significant increase of states who have individual child
record system since the last information collection package.
