The Small Health Care Provider Quality Improvement Program supports three years of funding to assist rural health care providers with implementation of quality improvement activities in a primary care setting. This requested data collection provides HRSA with information on grantee performance for meeting the goals of the program’s authorizing legislation. Respondents would be Small Health Care Provider Quality Improvement Program grant recipients.
