CLOSE INDIVIDUAL FISHING QUOTA (IFQ) ACCOUNT REQUEST
Instructions: Please complete this form if you want to close your IFQ online account. This form must
be completed and signed by all holders named on the IFQ shareholder or dealer account. If an IFQ
account holder is deceased, then a death certificate must also be submitted with this form. Shareholder
accounts cannot be closed until all shares and allocation have been transferred to another IFQ
account. Dealer accounts cannot be closed until all cost recovery fees have been paid. Please
complete and mail this form to:
