The FEMA form numbers has not been changed however, FEMA Form 119-17-1 and FEMA Form 119-17-2 has been revised. See Narrative of Form Changes provided for FEMA Form 119-17-1 and FEMA Form 119-17-2 uploaded in ROCIS.
