This form is used to appoint qualified
U.S. citizens as uncompensated registrars to assist in the
registration requirements of the Military Selective Service Act.
While the information collected is the same, the previous
collection had 6 separate forms--one for each group. The form has
been streamlined for the public to use so that there are not 6
separate forms but rather one form with the same information
collected and all of the 6 groups are listed on the same form. The
information collected is the same. The form includes the following
groups: High School , Federal Bureau of Prisons, Workforce
Innovation and Opportunity Act, State Correction Institutions
,National Farm-Workers Job , and Other. Please note that the
Workforce Investment Act was reauthorized by Congress in 2014 as
the Workforce Innovation and Opportunity Act. The last approved
collection listed it as the Workforce Investment Act.
