Dear [Name],
Thank you very much for agreeing to participate in the UCSF Medical Center Falls Prevention Program Survey being conducted on behalf of the Centers for Disease Control and Prevention (CDC). The purpose of this study is to collect information about the processes and effectiveness of the recent UCSF falls prevention program and interventions applied to at-risk adults 65 years and older who are hospitalized in an inpatient environment for 48 hours or longer but are within 18 hours of hospital discharge. Your participation in this important quality improvement initiative is greatly appreciated.
Within the next week, you will receive a brief survey. At your convenience please complete and submit the brief survey, which we anticipate will only require about seven (7) minutes of your time.
If you have any difficulties with the survey tool/link or need clarification about the survey questions, please contact Laura Gargano, Project Manager for Intellix/UCSF Falls Prevention Program, at 510-725-5363 or via email at lgargano@intellixs.com.
Thank you in advance for your time and participation in this important study.
Sincerely,
Gwen Bergen, PhD, MPH, MS
