NOTICE [n] – Information contained on this form that would permit identification of any individual
or establishment has been collected in accordance with Section 308(d) of the Public Health
Service Act (42 USC 242m), the Confidential Information Protection and Statistical Efficiency Act
(44 USC 3501 note), and the Privacy Act of 1974 (5 U.S.C. § 552a). In addition, NCHS complies
with the Federal Cybersecurity Enhancement Act of 2015, which requires the federal
government to protect federal computer networks by using computer security programs to
identify cybersecurity risks like hacking, internet attacks, and other security weaknesses. The
information collected will be used only for purposes stated for this study. Public reporting burden
of this collection of information is estimated to average about an hour per response, including the
time for reviewing instructions, searching existing data sources, gathering and maintaining the
data needed, and completing and reviewing the collection of information. An agency may not
conduct or sponsor, and a person is not required to respond to, a collection of information unless
it displays a currently valid OMB control number. Send comments regarding this burden estimate
or any other aspect of this collection of information, including suggestions for reducing this
