Recently we sent you a survey seeking information about the healthcare experiences of your child with a congenital heart defect. New York State Department of Health researchers hope to use responses from this survey to develop educational modules for adolescents with congenital heart defects, their families and their healthcare providers.

We have not yet received your completed survey. If you have already mailed this back to us, please accept our sincere appreciation. If not, we would please encourage you to do so. It is important that your answers be included.

We have included another copy of the survey for your convenience. Please complete it and mail it back in the pre-paid, pre-addressed envelope enclosed.
