Interim Performance Report OMB No. 0581‐0240
The interim report provides a status update regarding the progress made toward the objectives of your grant proposal. Particularly, recipients are expected to provide both qualitative and quantitative results to convey the activities and accomplishments of the work.
The report is limited to 6 pages and is due within 30 days of the project’s midpoint in the case of a planning grant, or every 6 months in the case of an implementation grant. Provide answers to each question, or answer “not applicable” or “no results available yet” where necessary. It is recommended that you email or fax your completed performance report to FMPP staff to avoid delays. The time required to complete this information collection is estimated to average 4 hours per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information.
