The Survey is a nationwide survey of enrolled Veterans conducted every year.
More than 42,000 Veterans will participate in the survey.
The survey will ask questions about your use of health care, insurance benefits, and
health status.
Your answers will help the VA plan for the future. VA uses this information to improve
services and plan better programs for people who need them.
Why was I selected?

You were selected at random, from among all Veterans enrolled in VA health care
services.
Because you were scientifically selected for the survey, your responses will represent
the experiences of many other Veterans throughout the country.
What would you like me to do?

We would like you to complete a survey. The survey will take about 20 minutes to

The study is being conducted by the Veterans Health Administration (VHA) Office of the
Assistant Deputy Under Secretary for Health for Policy and Planning.
This office serves as an advisor on VHA policy, strategic planning, and forecasting on the
demand for health care services.
Westat is under contract to VHA to collect data for the survey.
