Last week a questionnaire was mailed to you because your household was randomly selected to participate in the FDA Health and Media Study being conducted in 30 cities across the United States.
If you have not already completed the questionnaire, please have an adult household member fill it out and mail it to RTI International to determine your household’s eligibility in the study. Please keep the $2 enclosed in your questionnaire package as a token of our appreciation.
