Student Survey
NOTE TO REVIEWER: This survey will be administered on-site to students in grades 6 – 12 at the end of
their field study. A separate survey will be created for each of the 5 core modules of BTW. The first
section of the survey will be the same for all 5 modules.
We hope that you will complete this 4-minute survey telling us your thoughts about the field study
and what you learned. We will use the survey results to plan future field studies. Your responses will
be completely anonymous meaning that no one will be able to match you to your responses. When
you finish, please give your survey to the park educator. Thank you!
Paperwork Reduction Act Statement: The Paperwork Reduction Act requires us to tell you why we are collecting this
information, how we will use it, and whether or not you have to respond. The data collected in this study will assist mangers in
understanding local middle and high school students to assess the quality of the NPS & Alice Ferguson Foundation Bridging the
Watershed (BTW) Program. Your input will be critical in understand the utility of the program. Your responses are voluntary
and anonymous. Your name and address will not be collected. At the completion of this collection all personal information will
be destroyed and in no way be connected with the results of this survey. A Federal agency may not conduct or sponsor and you
are not required to respond to, a collection of information unless it displays a currently valid OMB Control Number.
Estimated Burden Statement: Public reporting for this collection of information is estimated to be no more than four minutes
to complete the questionnaire or less than two minutes to answer the non-response questions. Send comments regarding this
burden estimate or any other aspect of this collection of information, including suggestions for reducing this burden, to the

The BTW educator will group non-respondents and will read the following text aloud to them:
I would like 1 minute of your time to ask you for your response to the field study. I will read 2 statements
about the field study. Indicate your response to the statements by placing an ‘X’ in the box under ‘Yes’ or
‘No’. There is a 3rd statement that asks you why you didn’t complete the full survey.
Let’s begin.
[The BTW educator will read aloud the survey items in the table below pausing for 3 seconds to allow
non-respondents time to select their answer before moving to the next statement.]
