Industry Fall HPAI Planning Survey

1. Introduction
According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person
OMB
is not required to respond to, a collection of information unless it displays a valid OMB control number.
Approved
The valid OMB control number for this information collection is 0579-XXXX. The time required to
0579-XXXX
complete this information collection is estimated to average 2 hours per response, including the time or
Exp. XX/XXXX
reviewing instructions, searching existing data sources, gathering and maintaining the data needed and
completing and reviewing the collection of information.

To help APHIS prepare for a potential HPAI outbreak this fall, please complete this survey by
August 14, 2015. Data from the survey will help APHIS assist your industry if an outbreak occurs.
Specifically, the survey will ask you to describe steps taken by you to prepare for an HPAI outbreak
this fall. Please provide as much information as possible.
The questions marked with an * must be completed. If you don’t know an answer select “Don’t know”
on the multiple choice questions; on the write-in questions write “n/a” or “Don’t know” if applicable.
Survey Monkey when accessed through the web link will not allow you to save and return to the
survey to edit or complete later. Therefore, you may want to print the pdf version and gather the
needed information before starting the survey. If you would like to have the ability to save and return
to the survey you may request a personal web link to the survey by emailing
Randy.D.Munger@aphis.usda.gov.

Moving between pages in the survey should only be performed by using the Previous and Next
buttons located at the bottom of each page. Do not use your web browsers back button as you could
lose all your answers.
All responses must be finalized and submitted byAugust 14, 2015.
If you have questions about this survey please contact Diane Sutton at 301-851-3509 or at
Diane.L.Sutton@aphis.usda.gov. If you have technical problems with the survey please email
Randy.D.Munger@aphis.usda.gov.

To receive HPAI reports should new cases occur in the U.S. pleasesubscribe to the VS Animal Health
Stakeholder Registry. For more information on HPAI and biosecurity go to theAPHIS website.

32. End of Survey
Thank you for helping APHIS prepare for a potential HPAI outbreak in the fall. Responses to this
survey will help APHIS better understand how we can assist your industry and will also highlight
areas on your operation in need of improvement. Your involvement is critical to preventing HPAI
infection in your flocks and ensuring a swift response to any future outbreaks.

To receive HPAI reports should new cases occur in the United States pleasesubscribe to the VS
Animal Health Stakeholder Registry. For more information on HPAI and biosecurity go to theAPHIS
website. If you have any questions about this survey, please contact Diane Sutton at 301-851-3509 or
