ANALYSIS OF OFFICERS FELONIOUSLY KILLED AND ASSAULTED
This report is authorized by law Title 28, Section 534, U.S. Code. Please use this form to report
circumstances and other details regarding law enforcement officers from your department who were
feloniously killed or assaulted and injured with a firearm or a knife/other cutting instrument. The
information you submit will assist the FBI in the compilation of the annual publication, Law Enforcement
Officers Killed and Assaulted, and will also provide valuable data for law enforcement purposes, including
officer training. The anticipated release date of the annual publication is October of the year following the
year of death or assault. Previously released annual publications may be accessed on the Internet at
. Your cooperation, time, and effort are appreciated.

officer or attach a copy of the written summation of the initial incident report. Also, if the offender(s) was located or
apprehended later, please provide additional details, such as, when, where, and by whom was the offender located.
This narrative can provide pertinent details that may be incorporated into officer training programs and are often
difficult to capture in a “question and answer” format. The success of our endeavors to prevent further line-of-duty
deaths/assaults depends largely on the quality of data obtained from the victim officer’s agency.

HARRISON COUNTY HOMICIDE INVESTIGATION
SUMMARY REPORT
1. The suspect in this investigation is John Paul Smith, a white male, date of birth June 1,
1990, social security number 123-45-6789, address 100 East Main, Clarksburg, West
Virginia.
2. The violations of law are: Second Degree Murder (2 counts), a class A felony, Possession
of a Controlled Substance (Prior/Persistent Offender), a class C Felony, Unlawful Possession
of a Firearm, a class C Felony, and Resisting Arrest.
3. The violation occurred on September 4, 2014, at approximately 0021 hours, on Pike Street
near the intersection of Elm Street, Clarksburg, Harrison County, West Virginia.
4. The victims in this investigation are: Deputy William P. Jones, date of birth November 29,
1988, Jefferson L. Patrick, date of birth March 23, 1985, and the State of West Virginia.
5. A chronological synopsis of the investigation is as follows:
A. On September 4, 2014, at approximately 0021 hours, Deputy William Jones, Harrison
County Sheriff’s Office, attempted to stop a blue, 1988 Buick Skylark, for an equipment
violation (headlight out) on Pike Street, west of West Virginia 50, in Clarksburg, West
Virginia. Riding with Deputy Jones at the time was Donald Harris, a Harrison County
Sheriff’s Office Dispatcher. The occupants of the vehicle were later identified as, John
Smith, the driver, and Jefferson Patrick, the passenger. Smith failed to yield to Deputy Jones
and he notified Harrison County Dispatch he was in pursuit. The pursuit traveled westbound
on Pike Street to the intersection of Elm Street, at which point Patrick exited the Buick and
