School Agreement to Participate in a Research Study

As the authorized official of __________________school, I am agreeing to the participation of this school in the study entitled Understanding the Impact of Providing Information to Parents about the Role of Algebra II: An Opportunistic Study being conducted by REL Southwest.

I have been given a full description of the project and understand that school personnel will be asked to perform the following functions:

• Distribution of informational materials to students via mail

I understand I will be provided with a report on the outcome of the study upon approval from the U.S. Department of Education. The school system will also receive reimbursement for the cost of staff time and mailing materials, up to $150.

I understand that I may withdraw the school’s participation at any time.
