BP2 Data Display Survey

The purpose of this survey is to gather feedback on your thoughts around the BP2 Data Display Report.  We are collecting this data to design the BP3 Data Display Report.  This survey will address the report's overall utility, visual aspects, and data related content. 


In responding to the questions, it may be helpful to have a copy of the BP2 Data Display Report, mailed out by SHARPER on November 17, 2015.

Responses to this survey are voluntary. The survey should take approximately 15 minutes to complete.
