These forms serve as the means by
which borrowers in the William D. Ford Federal Direct Loan (Direct
Loan), Federal Family Education Loan (FFEL) and the Federal Perkins
Loan (Perkins Loan) Programs may request forbearance of repayment
on their loans if they meet certain conditions. The U.S. Department
of Education and other loan holders uses the information collected
on these forms to determine whether a borrower meets the
eligibility requirements for the specific type of
forbearance.

This is a request for a
revision of the current information collection 1845-0018. There is
a minor increase to the burden hour of 579 hour and an increase of
1,200 responses by including the Perkins loan borrowers on the
forms. There are no changes to the information collected on these
forms, however, the forms have been simplified and reformatted to
present a common layout for these types of requests. There has been
no change in the underlying statute or regulations which support
thee request forms.
