TABLE OF CHANGES - FORM
Form I-90, Application to Replace Permanent Resident Card
OMB No 1615-0082
11/20/2014
Reason for Revision: USCIS is revising the words “re-verify” and “re-verifying” in the Acknowledgement of Required Appointment at USCIS Application Support Center, Applicant Certification, and Interpreter Certification sections of the Form I-90 to read “re-affirm” and “re-affirming.” This change is necessary to properly reflect what an individual is required to do at an ASC appointment. Form I-90 applicants are not physically reviewing a copy of their applications during an ASC appointment. They are simply re-affirming that the Form I -90 they signed and filed with USCIS remains complete, true and correct as they originally certified and attested to, under penalty of perjury, at the time of filing the application. The Department of Justice has approved this wording change.
