Introduction over the Phone for Targeted Participants for the HIV Risk Factor Study

This is ______ calling from ____ Blood Center. You are invited to take part in a survey, which is jointly conducted by __________ Blood Center, Institute of Blood Transfusion (of Chinese Academy of Medical Sciences), the Johns Hopkins School of Medicine and the United States National Institutes of Health. The objective of this survey is to learn about the risk factors for HIV infection among blood donors.

Your participation is voluntary. You have the right to withdraw at any time. If you agree to participate, we will mail you a study packet which includes a consent form, a questionnaire, and a pre-stamped envelope for returning the completed questionnaire. Once we receive your completed questionnaire you will receive a 100 yuan monetary reward for participating in the survey. I want to confirm your agreement and then, if you agree, your correspondence address before we send out the packet.
