On <date>, you received a letter and a brochure describing the Summer Food Service Program (SFSP) Participant Characteristics Study and how important your participation is in this study.

As a participant, you are invited to take a survey that collects information regarding your participation in the summer foods program, such as attributes of your site and the children who visit your site for summer meals. To complete the survey, please follow the link below:
