School Level Survey Recruitment Script

We are conducting a national evaluation of the implementation of Safe Schools/Healthy Students
(SS/HS) programs. This evaluation will help improve the implementation and provision of mental health
services in schools and communities. You have been identified based on your involvement in the
provision of SS/HS programs within your agency. Because of your position in this program, we would
like to ask if you would be willing to participate in a short survey regarding the implementation of your
SS/HS program.
The survey includes questions about developing and implementing evidence based services for children
and youth as part of the federal grant, as well as any successes and challenges to the effective provision
of services, and recent efforts to improve the coordination of services and collaborations among
schools, communities and families. The survey will take approximately 25 minutes to complete and can
be completed online by following the link provided below in this email. All data obtained from this
survey will be kept confidential and will only be reported in an aggregate format. No names will be
collected, nor will any identifying information be associated with your responses. Participation in this
study is completely voluntary and you will have the right to stop the survey at any time or refuse to
participate entirely.
