To improve usability, data quality, and respondent satisfaction in completing the assessment, CDC desires to delete four duplicative tables from the State and Community Awardee Performance Measure Reporting Tool, The Program also wishes to add an additional column to one table and add a question to the same form. This information was already being gathered and the actions result in a decrease in burden.

Program has made changes to the State and Community Awardee Performance Measure Reporting Tool. Four Tables were found to be redundant and were removed. One column was added to a Table and one additional question is being asked. This does not affect the overall scope of the information collection and results in an overall decrease in burden for this form from 4 to 3.25 burden hours.
