Dear [ PRINCIPAL INVESTIGATOR ]:


On behalf of the Native Diabetes Wellness Program, I recently sent a letter to you concerning CDC’s plan to collect summary information about “Traditional Foods and Sustainable Ecological Approaches for Health Promotion and Diabetes Prevention in American Indian / Alaska Native Communities.”


We’re excited to learn more about the Traditional Foods activities in your community. We hope that the Shared Data Elements (SDE) help the NDWP staff communicate with you, help Traditional Foods awardees share successes with each other, and help CDC learn how it can provide meaningful support to community-based health promotion programs like yours.


Below is a link to the website where you will enter your community’s Shared Data Elements information. The SDE website was constructed using Survey Monkey, and also contains a link to the privacy policy. You will submit SDE two times per year. The next transmission is due [ mm/dd/yyyy ]. Your username is [xxxxxxxx] and your password is [xxxxxxxx].

After we receive SDE from all Traditional Foods awardees, we will send you a summary report so that you can see the variety of innovative activities occurring in AI/AN communities. This will be a statistical summary based on aggregated data. We do not plan to identify individual awardees in this report.
