Application for Death Benefits under
the Federal Employees Retirement
System, is needed to collect information
so that OPM can pay death benefits to
the survivor of Federal employees and
annuitants. SF 3104B, Documentation
and Elections in Support of Application
for Death Benefits When Deceased Was
an Employee at the Time of Death, is
needed for deaths in service so that
survivors can make the needed elections
regarding health benefits, military
service and payment of the death
benefit.

Annuity Supplement Earnings Report.
As required by the Paperwork
Reduction Act of 1995 (Pub. L. 104–13,
44 U.S.C. chapter 35) as amended by the
Clinger-Cohen Act (Pub. L. 104–106),
OPM is soliciting comments for this
collection. The Office of Management
and Budget is particularly interested in
comments that:
1. Evaluate whether the proposed
collection of information is necessary
for the proper performance of functions
of the agency, including whether the
information will have practical utility;
2. Evaluate the accuracy of the
agency’s estimate of the burden of the
proposed collection of information,
including the validity of the
methodology and assumptions used;
3. Enhance the quality, utility, and
clarity of the information to be
collected; and
4. Minimize the burden of the
collection of information on those who
are to respond, including through the
use of appropriate automated,
electronic, mechanical, or other
technological collection techniques or
other forms of information technology,
e.g., permitting electronic submissions
of responses.

will be accepted until October 15, 2012.

SUPPLEMENTARY INFORMATION: RI 92–22,
Annuity Supplement Earnings Report, is
used each year to obtain the earned
income of Federal Employees
Retirement System (FERS) annuitants
receiving an annuity supplement. The
annuity supplement is paid to eligible
FERS annuitants who are not retired on
disability and are not yet age 62. The
supplement approximates the portion of
a full career Social Security benefit
earned while under FERS and ends at

age 62. Like Social Security benefits, the
annuity supplement is subject to an
earnings limitation.

B and C appointing authorities
applicable to a single agency are not
codified in the CFR, but the Office of
Personnel Management (OPM)
publishes a notice of agency-specific
authorities established or revoked each
month in the Federal Register at
www.gpo.gov/fdsys/. OPM also
publishes annually a consolidated
listing of all Schedule A, B and C
appointing authorities current as of June
30 as a notice in the Federal Register.
