High Cost Broadband Data Submission Survey


The Universal Service Administrative Company (USAC) seeks your input on an important part of the modernization to the Universal Service High Cost program resulting from the Rate-of-Return Reform Order. USAC is building a new online system that will serve as a repository for the geospatial broadband deployment data that must be filed by all carriers that receive Connect America Fund (CAF) support. This data will show the exact locations (by longitude and latitude) where carriers receiving Universal Service CAF support are building out their networks. We expect the new online system will be up and running — and ready to accept location-based deployment data from carriers — by January 1, 2017. As USAC begins the process of designing and building the system, we are engaging with the carriers and consultants who will use this tool to submit deployment data in order to better understand how the data will be collected and submitted so that we can build a system that integrates with other systems. Please help us by answering the questions in the attached survey. We want your input on what the system should look like, how it should work and what functionality it should offer so that it will meet your needs. Your insights and feedback are important to us. This survey is voluntary and consists of 28 questions, and it should take approximately fifteen minutes to complete.
