Notes to Interviewer:
This protocol will be used to guide open-ended check-in telephone interviews with participants involved in responsible fatherhood programs. Depending on the father’s responses to his previous interview and his experiences since then, not all of the questions in the protocol will be relevant. For that reason, the interviewer will tailor the questions to each participant.
This interview is expected to last 5 to 10 minutes.
Introduction – Consent Statement
Hi! I’m _________, the interviewer who talked with you at [PLACE] in [MONTH]. As you probably remember, I work for Mathematica, an independent social policy research firm. We are conducting a study sponsored by the U.S. Department of Health and Human Services on fatherhood programs, which includes talking to fathers in different areas of the country who have participated in these kinds of programs.
Today, I am just calling to check in with you. We’re scheduled to meet again in person in six months but meanwhile, we would like to check in and see if there have been any changes in your life that have occurred since we last talked. Just like the earlier interview we did with you in person, we will not be using your name or any other identifying information in reports, so your comments will not be linked to you.
The call will take about 5 to 10 minutes, and your participation is completely voluntary. No one may conduct or sponsor a collection of information unless it has a currently valid OMB number; the OMB number for this collection is: 0970-0403. If you have any questions, please contact [CONTACT NAME], at xxx-xxx-xxxx, regarding OMB-PRA (0970-0403).
