On the previous form (2/3/2011), #7 and #9 of the “Instructions” section were the exact same sentence:
“If subsequent action is taken to seal or expunge record, attach certified or authenticated copy of court order to this form.”
Therefore, we removed #9.
Also, while looking at this form, I made some minor changes to formatting.
Under the portion labeled “Subject’s Relationship to Victim,” I did not edit the language but made sure everything lined up so it was easier to read.
Also, I removed the extra period in the quotation found in the top portion of the form since it was a typo.
The main reason for the update was the duplicative sentence issue. I just figured I would look over the other sections too.
