Edits to 2900-0554 Instrument Covers/Title Pages and Assurances pages


This collection request was first associated with Rulemaking 2900-AN81, 77 FR 12697, 03/01/2013. A NOA was issued with an expiration date of 12/31/2012. Ms McLamb resubmitted the -0554 data collection as an extension, under the impression that the NOA date would cause the collection to expire.


During the standard OMB conference call/review, the VHA program POC’s made note of an oversight on their part. VHA no longer uses VA Form 10-0361LSC. Therefore, the burden hour was reduced. Stephen Shaw made edits to the justification and asked that VHA update ICR statements, as noted below.


  1. Stephen Shaw edited supporting statement no. 12 by removing all reference to 10-0361-LSC. VHA no longer requires application provisions for the Life Safety Code capital
    grants form.


  1. The removal of form 10-0361 LSC, resulted in an ICR burden reduction “Revision.”


  1. A couple of Per Diem cover pages did not have accurate burden hours in the PRA statement. Two covers had the wrong burden hour of 35, which covers the entire collection burden. It was corrected to reflect the accurate burden for each document.


  1. ROCIS: Instrument File: The titles referencing ”83C NEW 2006 PDO…” were updated, removing “83C New 2006.”


  1. All item B “Assurances” Reports; Record Retention paragraphs with the following excerpt were edited to include the text highlighted in yellow:

If this proposal is funded, applicants assure that any and all reports required by the Secretary of Veterans affairs shall be made in such form and contain such information as the Secretary may require and/or are required by law. Applicant further assures that upon demand, the Secretary of Veterans Affairs has access to the records upon which such information is based.
