We are writing to ask for your participation in a web-based survey being conducted on behalf of the Centers for Disease Control and Prevention (CDC) by RTI International. As referenced in the e-mail you received on DATE from Angela Moore, MPH, Comprehensive Cancer Control Branch (CCCB) Program Evaluation and Partnership Team Lead, this survey is one component of the NCCCP evaluation. The purpose of the survey is to understand what CDC can do to help grantees in their efforts to implement the NCCCP Priorities, and to gather information about use of the CCCB Program Evaluation Toolkit.
