Recordkeeping: Notification

A. Recordkeeping: Notification of Arriving Shipments

1. The importer’s name and address;

2. Number and species of NHPs being imported;

3. Description of crates;

4. Means of individually identifying NHPs;

5. Origin of NHPs, including the country, the exporter, and the exporter’s address;

6. Intended use of NHPs;

7. Specific itinerary with names, dates, flights, times, airports, sea ports, and responsible parties to contact at every step of travel, including all ground transportation;

8. Port of entry;

9. If arriving by flight, the name of the airline and its flight number;

10. If arriving by vehicle, the name of the vehicle’s owner and its license plate number;

11. If arriving by ship, the name of the ship and its vessel number;

12. Name and address of the destination quarantine facility;

13. Name, address, and contact information for shipper, if other than the importer;

14. If applicable, name, address, and contact information for broker in the United States;

15. Name, address, and contact information for the person(s) responsible for off- loading NHPs in the United States;

16. Name, address, and contact information for any party responsible for ground transportation from port of entry to quarantine facility;

17. Expected quarantine facility, if different from the importer;

18. Master air waybill number for shipment;

19. CITES permit number and expiration date.

20. Post arrival: any deaths during shipping, including cause of death
