Reason for Revision: USCIS is revising the template of the form to make it more user-friendly and easier to complete, as well as to accommodate enhancements to USCIS intake systems for Lockbox processing. USCIS does not anticipate that the revisions will result in any increase in time burden to complete the form as the main focus was reformatting to provide clarity for the applicants.
